The National Association of Chronic Disease Directors (NACDD), in collaboration with the Centers for Disease Control and Prevention (CDC) NCCDPHP* Division of Population Health – Arthritis Program, announce the availability of funds for any organization able to increase participation in the Arthritis Foundation Walk With Ease (WWE) group and self-directed interventions. Walk With Ease is a six-week program offered in two formats – instructor-led classroom format and the self-directed format. Walk With Ease programs are designed to increase physical activity, reduce pain and improve quality of life for adults living with arthritis.
NACDD will award approximately $6,000, to organizations able to coordinate sub-awards, recruit and support participants as well as oversee implementation of activities to increase awareness, enrollment and participation in the WWE self-directed intervention and group intervention. Each grantee will be expected to have a collective total of 1,500 individuals or more complete the WWE intervention during the project period.
Project period will from May 17, 2019, through September 29, 2019. NACDD and the Arthritis Foundation will work closely to assist selected applicants with project implementation.

Applicants must present a plan for sustaining implementation of the selected Walk With Ease format after grants funds are no longer available. Applicants can propose fundraising and revenue-generating strategies to continue services.
